Statewide Subscribers needing to replace one affiliate institution with another should complete and submit the form below.

This is your official recognition and communication to QM that the new affiliate is now entitled to the benefits of your existing subscription agreement with MarylandOnline (QM) and that the access of the previous institution to the same will be removed.
