If your institution's representative to The Quality Matters Program has changed, or you wish to designate an additional representative, please complete and submit the form below.

To schedule training for the new representative, please refer to the online training calendar. 

Institution Representative:

- Will be Quality Matters' liaison within the institution

- Should be the individual with day-to-day responsibility for implementing Quality Matters within the institution

- If desired, institution may add a separate contact for contract matters
