Please complete and submit the following form to formally notify QM of your intent to subscribe.

The Institution Representative will receive a Quality Matters Program Subscription Agreement and Invoice. Subscription is not confirmed and services will not begin until signed contract is returned. Payment is due within 30 days upon receipt of invoice.

Single School - Basic Subscription - $1650

Single School - Full Subscription Standard (3 or more online programs) - $3300

Single School - Full Subscription Small Program (2 or fewer online programs) - $2200

Consortium - Full Subscription Standard (3 or more online programs)- $2475 per institution

Consortium - Full Subscription Small (2 or fewer online programs) - $1,650 per institution

Statewide - Full Subscription ($5,500 for the lead agency/institution plus $1,100 per affiliate, regardless of size)
