Publisher Intent to Subscribe or Sponsor

Please complete and submit the following form to formally notify QM of your intent to subscribe or sponsor.

The Publisher Representative will receive a Quality Matters Program Subscription Agreement and Invoice. Subscription is not confirmed and services will not begin until signed contract is returned. Payment is due within 30 days upon receipt of invoice.

Publisher Representative:

- Will be Quality Matters' liaison within the institution

- Should be the individual with day-to-day responsibility for implementing Quality Matters

- If desired, the publisher may add a separate contact for contract matters
